00:00:00
Health is not an accident.

00:00:02
You don't get there by guessing, by just trying things.

00:00:06
It has to be engineered and engineering requires data.

00:00:09
It really does.

00:00:10
Not fragments, not guesses.

00:00:12
Let's see what happens if I try this.

00:00:13
That's how people spin their gears and waste time and money.

00:00:16
Test, map, treat, retest, optimize.

00:00:21
And that is really the future of medicine.

00:00:23
Welcome to Health Decoded, your number one source for real health information with your

00:00:28
host, doctor Alessi and Matt Tack.

00:00:34
Welcome back to another episode of Health Decoded.

00:00:37
Today, are gonna talk about something really cool that I believe represents the future of

00:00:42
medicine and quite frankly, the dividing line between reactive health care and true health

00:00:49
restoration as we call it.

00:00:51
And the topic here is the five lab tests you need but probably have never done and your

00:00:58
doctor isn't going to do for you.

00:01:00
Let me say something upfront here that may challenge how you think about healthcare

00:01:05
overall.

00:01:06
And that is that standard labs are really becoming a thing of the past.

00:01:11
Not because they're useless, but because they're incomplete.

00:01:15
They were designed for a medical system whose primary objective is to detect disease once

00:01:20
it's already developed.

00:01:22
But what if your goal isn't just to avoid disease?

00:01:25
What if your goal is resilience, longevity, metabolic strength?

00:01:30
What if your goals are cognitive clarity and hormone stability or a strong immune system?

00:01:37
Quote me on this, You cannot build elite health with limited data, and I will stand by that.

00:01:42
And yet, single day I meet intelligent, motivated people who tell me, My doctor said

00:01:48
everything looked normal, doc, but they're exhausted, they're inflamed, they're

00:01:52
struggling, they're mentally foggy, they know something's not right.

00:01:56
So let's offer maybe a simple analogy here.

00:02:00
Waiting for standard labs to catch a problem is like waiting for your car engine to cease

00:02:06
before you decide to change the oil.

00:02:09
Maintenance always beats breakdown.

00:02:11
So waiting for something bad to happen isn't always the best approach.

00:02:15
So comprehensive testing like we do here at Ascend Functional Health is what we call

00:02:20
maintenance medicine.

00:02:22
If you're not running these deeper tests, you are not practicing proactive healthcare.

00:02:27
You are waiting for something to go wrong.

00:02:29
And so, today I'm going to walk you through the five lab tests that allow us to map the body's

00:02:35
internal ecosystem and what I often call biological terrain mapping.

00:02:40
Because if you cannot clearly map what's going on, you are not ready to treat.

00:02:45
And so let's get right into it because we're going talk about a few pretty deep topics here.

00:02:51
So I don't want to make this episode too long.

00:02:53
So, lab number one.

00:02:55
Okay?

00:02:55
And this is the one that we run on basically every single patient that comes into the

00:03:01
office, our comprehensive panel.

00:03:03
Now, this is a customized test that I formulated myself based on what I've seen I

00:03:10
need to fully understand the ecosystem inside somebody's blood and what things we need to do

00:03:17
and how we can make the right moves and changes going forward.

00:03:21
And so everything begins here.

00:03:23
Before we begin optimizing before peptides and advanced protocols, have to understand the

00:03:28
terrain.

00:03:29
And so a true comprehensive panel is not just a CBC and cholesterol, which you're going to get

00:03:35
at your standard doc.

00:03:37
It's a multi system physiological snapshot.

00:03:40
So let's walk through what that means.

00:03:43
Foundational chemistry.

00:03:44
We evaluate the CBC and comprehensive metabolic panel alongside electrolyte levels, looking at

00:03:52
oxygen delivery, immune patterns, kidney and liver function, as well as your cellular

00:03:57
hydration.

00:03:58
Because subtle shifts here often will precede disease by years.

00:04:04
We look at what we call cardiac intelligence.

00:04:06
We're not guessing if you're going to have a heart attack.

00:04:09
We're not saying, Yeah, maybe.

00:04:11
Right?

00:04:11
It's going be okay.

00:04:13
We analyze advanced markers including, and these are the ones that you will not get at

00:04:19
your standard doc, a PLA C score, this is basically a cardiac artery plaquing score,

00:04:25
homocysteine, an inflammation marker, oxidized LDL, myeloperoxidase or MPO, HSCRP, and then NT

00:04:34
proBNP.

00:04:35
These are all myocardial risk inflammation factors.

00:04:40
And so these markers will reveal inflammation inside the vascular system long before a

00:04:45
blockage occurs.

00:04:47
Heart disease is not sudden.

00:04:48
We know that by now.

00:04:49
It is quietly brewing for decades until something triggers and someone has a heart

00:04:55
attack.

00:04:56
The next thing we look at is hormonal architecture.

00:04:58
We assess total and free testosterone, sex hormone binding globulin, is your body

00:05:04
utilizing these androgens, estradiol, DHEA, cortisol, progesterone, FSH and LH.

00:05:12
And because hormones do not just influence reproduction, they actually regulate your

00:05:17
metabolism, your brain function, sleep, recovery, and motivation.

00:05:22
They operate in a cascade where if one thing is off, the body's going to prioritize producing

00:05:27
other hormones and we can create an imbalance over time often due to stress and lack of

00:05:33
intervention.

00:05:34
The next thing we look at is thyroid, basically your metabolic governor.

00:05:38
And so we're going to evaluate TSH, T3, T4, reverse T3, but we also always include anti TPO

00:05:46
and anti TG markers for autoimmune risk because the thyroid dysfunction is often missed when

00:05:53
practitioners rely solely on TSH and T3.

00:05:56
They're only getting a piece.

00:05:58
We look at glycemic control.

00:06:00
So we're gonna analyze fasting glucose, insulin, hemoglobin A1C, beta cell function and

00:06:06
ferritin.

00:06:07
Insulin resistance begins silently.

00:06:09
We often miss, Patients often miss this ten to fifteen years before they develop diabetes,

00:06:14
they had some level of insulin resistance building.

00:06:17
That goes into our marker that we test which is an HOMA IR.

00:06:22
It's basically an algorithmic score of your body's insulin resistance developing.

00:06:27
And so, we'll see somebody and I see this all the time.

00:06:30
I literally just saw two of these this week.

00:06:32
Patients who have normal glucose, normal fasting glucose, normal insulin and normal A1C,

00:06:38
but they have this high HOMAIR score basically telling me that the pancreas is working to

00:06:45
produce insulin and becoming more resistant over time.

00:06:48
Even if it's a moderate score, it tells me something under the surface is going on.

00:06:53
And then we look at nutritional markers as a part of this panel as well.

00:06:57
Folate, vitamin D, B12, and this is not optional.

00:07:01
These are foundational markers that give us the physiological map because this is the overall,

00:07:08
what I call it, widest net that we can cast to get the most information, the most efficiently,

00:07:15
if you will.

00:07:16
And so, this is the beginning because there's other tests that we're going to talk about now

00:07:22
that take it a little bit deeper.

00:07:24
But again, you are not getting these at your standard doc, your PCP with your checkup.

00:07:30
And I air quoted that.

00:07:32
And so the next test that we're going to run, and this is probably the one we run second

00:07:37
most, that's why it's number two, is our gut zoomer or our gut scan.

00:07:42
And so most chronic illness, and I will stand by this, most chronic illness has a gut related

00:07:48
component or origin.

00:07:51
The Gut Zoomer provides us with deep insight into your digestive and systemic symptoms by

00:07:57
analyzing the gut microbiome, pathogens, inflammation markers, and your overall gut

00:08:02
function.

00:08:03
It identifies markers linked to leaky gut, immune reactivity, poor detox capacity,

00:08:10
nutrient malabsorption.

00:08:12
All of these are drivers of chronic inflammation and hormone imbalance.

00:08:16
So, we're going to evaluate critical markers in this test, in particular zonulin and anti LPS

00:08:23
or lipopolysaccharide.

00:08:25
These are indicators of intestinal permeability or leaky gut.

00:08:29
We're gonna test for calprotectin.

00:08:31
This is a signal of inflammatory activity in the gut.

00:08:35
Beta glucuronidase which is tied to toxin and estrogen clearance.

00:08:40
And then short chain fatty acids which are essential for metabolic and gut lining health.

00:08:45
This test also goes very deep and very specific into the different microorganisms, both

00:08:52
probiotics, commensal biotics, pathobiotics, viruses, fungus, yeast.

00:08:59
And it's often the things that are missed in standard testing.

00:09:02
And again, those fungal, parasitic and viral contributors that can drive things like

00:09:07
fatigue, skin issues, and immune dysregulation.

00:09:12
And so the reality here, you know, clinically is you can't optimize your hormones, you can't

00:09:17
optimize inflammation, you can't optimize your brain function and energy with a dysbiotic gut

00:09:22
because that is built there.

00:09:25
Your neurotransmitters are built in the gut.

00:09:27
We've talked about this in previous episodes.

00:09:30
And so gut restoration often becomes the first domino, the first thing we approach.

00:09:35
Next, basically the test that we run-in conjunction with the gutsumer almost ninety to

00:09:40
ninety nine percent of the time is a food sensitivity test.

00:09:44
This pairs with the gutsumer.

00:09:47
Because now once we understand the gut, we have to understand also what's maybe triggering or

00:09:51
aggravating it.

00:09:52
The food sensitivity profile measures both IgA and IgG antibody responses to detect immune

00:10:01
reactions to commonly consumed foods.

00:10:04
So, these are delayed sensitivities unlike an allergy, which if you don't know the

00:10:10
difference, an allergy is an IgE response.

00:10:12
Your body has an allergic anaphylactic response to food.

00:10:16
These are severe.

00:10:17
These are things you need epinephrine for or even to go to an emergency room because it can

00:10:22
cause your throat to swell up or severe reactions.

00:10:26
Sensitivities are different.

00:10:27
Sensitivities are often under the scenes.

00:10:30
You don't even really notice it.

00:10:32
Maybe you get some digestive discomfort, maybe you feel brain foggy if you're very in tune,

00:10:36
but most people are not and they miss these.

00:10:38
And then we're eating these foods on a daily basis that are just triggering constant

00:10:42
reactivity and inflammation within the body.

00:10:45
And so this test will call that out.

00:10:48
Many patients are going to say, I tried the elimination diet and it didn't help.

00:10:53
Well, it's because you were guessing.

00:10:54
You were trying to figure out, oh, which things worked for me and which didn't.

00:10:57
This test actually analyzes your body's immune response to over 97 foods helping uncover

00:11:04
hidden drivers of inflammation and immune activity.

00:11:07
And so when we remove inflammatory triggers by eliminating the foods causing it, now we can

00:11:12
accelerate healing dramatically.

00:11:15
And so, let's get into the next test that we often run, the whole blood nutrient and omega

00:11:20
panel.

00:11:21
But real quick, before we do that, I want to take a quick word from one of our sponsors.

00:11:25
Hey, busy professionals and parents.

00:11:28
Let me ask you something.

00:11:29
Do you need help with your low energy, stubborn weight, brain fog, gut issues or hormones, but

00:11:36
you don't know where to start?

00:11:37
Well, you're not alone.

00:11:39
The biggest problem is that modern healthcare is designed to diagnose disease, but often

00:11:45
misses the dysfunction that begins years earlier as under the surface symptoms.

00:11:51
That's exactly why I created the Functional Medicine Starter System.

00:11:55
This is a structured, physician led process designed to uncover what is actually driving

00:12:01
your symptoms, whether it's rooted in gut health, metabolic function, inflammation,

00:12:06
nutrient status, or hormonal imbalance.

00:12:09
No guessing, no generic protocols, just real data and a clear plan.

00:12:14
So, you're tired of searching for answers, but you're not sure how to begin solving your

00:12:18
health issues, go to the link in the podcast show notes to learn more about the functional

00:12:23
medicine starter system.

00:12:25
Because once you have clarity, you can finally start to feel like yourself again.

00:12:30
All right, let's get back to the episode here.

00:12:32
Thank you for that.

00:12:34
So, the fourth lab that we often run and is a part of what we call our big five is the whole

00:12:39
blood nutrient and omega profile or the micronutrient profile or the nutrient zoomer

00:12:44
has a few different names.

00:12:46
And a mistake I constantly see is that people are taking supplements because someone told

00:12:51
them, Oh, this helped me.

00:12:52
Or because someone told them, Yeah, this one's good for you.

00:12:55
Or because their chiropractor, not to shed any shade here, but told them, Yeah, just take this

00:13:02
random supplement.

00:13:03
Without knowing if it actually was helping them.

00:13:05
That's not functional medicine, that's just guesswork.

00:13:09
The whole blood nutrient profile evaluates over 40 nutrients, including vitamins, minerals,

00:13:15
antioxidants, amino acids, to identify deficiencies that could be affecting your

00:13:20
health.

00:13:21
What makes this test powerful is that it measures both intracellular and extracellular

00:13:27
levels, offering a true snapshot of your nutrient status.

00:13:31
Why does that matter?

00:13:32
Because serum levels alone can look normal while the cell might be starving for nutrition.

00:13:38
So, deficiencies here are often linked to conditions like heart disease, osteoporosis,

00:13:44
neurological disorders, fatigue, mood changes, and one we see a lot is that impaired recovery

00:13:50
or impaired healing.

00:13:51
We'll have patients coming through here doing one of our physical medicine programs.

00:13:56
Maybe they're working on restoring an injured disc or a hurt, you know, an ACL or a knee or a

00:14:03
shoulder or whatever it might be.

00:14:05
And they're trying to heal and it's slow and it's taking forever and they have relapses.

00:14:09
And it's because their body's lacking the nutrition, the building blocks to allow them to

00:14:14
heal and recover.

00:14:16
And so when we lack those building blocks, performance will drop as well.

00:14:19
Recovery will drop and optimization obviously becomes impossible at that point.

00:14:24
So it's a very important test.

00:14:26
And then the fifth test on the list here that is something that I love and this is because

00:14:32
this is becoming more commonplace now with just the world we live in is the total toxicity

00:14:38
score.

00:14:39
And so basically we call it the total toxic burden test.

00:14:42
And this is going to check for basically every toxin that could be within your body, the total

00:14:47
toxic load we call it.

00:14:48
And so this panel analyzes excreted toxins, detox capacity, and oxidative stress to uncover

00:14:57
contributors to your fatigue, immune dysfunction, and cognitive issues.

00:15:03
It reports 87 urinary markers including 20 heavy metals, 29 mycotoxins or mold toxins, and

00:15:11
38 environmental toxins such as plastics and xenoestrogens that could be developing building

00:15:17
within your body.

00:15:19
We also assess detox pathways such as glutathione conjugation, sulfation, and

00:15:25
methylation.

00:15:26
All are essential for safe toxin clearance.

00:15:30
And so, here's the uncomfortable truth is you can take every supplement in the world, you are

00:15:36
not going to out supplement toxicity.

00:15:38
You just won't.

00:15:39
Even a lot of these over the counter Instagram ad detoxes or pills or binders or whatever you

00:15:48
call them.

00:15:49
If they're not done in an appropriate way, can actually make you worse because they're going

00:15:52
to pull toxins into the blood, but not actually help you clear them.

00:15:56
So, has to be done.

00:15:57
I always recommend this is done under the supervision of a professional who knows what

00:16:01
they're doing.

00:16:02
Like the doctors that ascend functional health.

00:16:05
And so, again, interference must be reduced before optimization can occur, as always.

00:16:13
And so, again, those are the big five labs we call them.

00:16:17
This is the optimal panel that we will run with the patients coming in here who are really

00:16:23
hoping to get ahead of things and really get to the highest level of health.

00:16:27
This isn't for everyone, most patients will start with either this or some level of this

00:16:33
and we'll get the overall snapshot of how we're going to help this person go from sick,

00:16:40
struggling, confused, to optimizing their health and living the life of their dreams.

00:16:45
And so, in closing here, let me just leave everybody with this thought.

00:16:50
And I would love, love, love if you found value in this.

00:16:53
If this seemed like something that could help someone yourself, please just follow the show,

00:16:58
subscribe to stay in touch, but also share this with somebody.

00:17:01
When we share these episodes, we're getting the word out there that there's more to medicine

00:17:06
than just waiting to get sick and it could help somebody.

00:17:09
Really could.

00:17:10
So, health is not an accident.

00:17:14
You don't get there by guessing, by just trying things.

00:17:18
It has to be engineered and engineering requires data.

00:17:21
It really does.

00:17:22
Not fragments, not guesses.

00:17:25
So not, Hey, let's see what happens if I try this.

00:17:27
Right?

00:17:28
That's how people spin their gears and waste time and money.

00:17:31
So in this order, test, map, treat, retest, optimize.

00:17:37
That's how we operate and that is really the future of medicine.

00:17:40
