00:01:49
Welcome back to another episode of Health Decoded.
00:01:53
I am your host, Doctor.
00:01:54
Alessi.
00:01:55
And today we have a really fun and a really cool episode.
00:01:59
I received a DM on Instagram from a listener of the show that had a very specific question
00:02:05
regarding some of the things we've talked about on other episodes and how they work together.
00:02:11
And that's awesome because that's the whole concept of functional medicine is how our body
00:02:15
systems all play in together and create the whole being of who we are as people, right?
00:02:21
And so, this is gonna be a little bit shorter, a little bit quicker, and a little bit
00:02:25
different than our standard episodes where we go through specific topics.
00:02:30
This one's gonna be focused on this question I received.
00:02:32
But essentially, this person reached out to me on Instagram and said, Hi, I'm curious in
00:02:39
learning more about PCOS, which is polycystic ovary syndrome or ovarian syndrome.
00:02:46
And more specifically, more she was interested in learning more about the insulin resistance
00:02:51
component to that.
00:02:52
And so, basically, I came up with kind of a little bit of a script here where I want to go
00:02:57
through some things, discuss what PCOS is, obviously discuss what is the connection with
00:03:02
insulin answering your question.
00:03:05
And then how do we test for this, what things we should look at, and then what we can do
00:03:09
about it, right and maintain health.
00:03:11
And so I'm going to kind of start going through this.
00:03:13
And, you know, if this is something that you've dealt with, or you know someone deals with,
00:03:17
please share this with them.
00:03:19
Let them kind of engage with this and see if this is something that rings for them.
00:03:23
And then what they need to do to obviously, know, work on getting towards a state of
00:03:27
balance in a state of health.
00:03:29
So, let's start with this and thank you again for this question.
00:03:33
I'm not going to name this person because just for the concept of anonymity here, but thank
00:03:39
you again for sending this in and we would like to do more of these.
00:03:42
So, if you do have questions, if you do have concerns or things that you'd like to hear
00:03:46
answered on the show, please send them in.
00:03:48
And so, let's talk about PCOS, polycystic ovarian syndrome.
00:03:53
So, despite the name, you don't need ovarian cysts to have this.
00:03:57
It's really a hormonal and metabolic condition and most commonly what it's going to present as
00:04:02
are things like irregular or absent periods, excess androgens like testosterone in females
00:04:09
which can lead to acne, hair growth or hair loss.
00:04:13
It's going to lead to things like ovulation issues, which can make fertility more
00:04:17
difficult.
00:04:18
And yes, you can get the stubborn cysts on the ovaries, but they are a symptom, not the root
00:04:23
cause.
00:04:25
And so, the underlying root for many women really is the insulin resistance component.
00:04:31
Now, let's get into this insulin connection and ultimately what that means.
00:04:36
So, as we've discussed in a few episodes previously, more recently actually, insulin is
00:04:42
your blood sugar storage hormone.
00:04:45
Think of it like the key that unlocks the cells to be able to take in fuel.
00:04:49
But when you become resistant to insulin, which is very common, your body keeps pumping out
00:04:55
more and more and more.
00:04:57
Now, this happens because when we have elevated blood sugar for too long, or consuming meals
00:05:02
too often, we have a bit of metabolic dysfunction, insulin is going to be secreted
00:05:07
more often, and higher dosages, over time the body becomes resistant to this.
00:05:13
So, high insulin does a few dangerous things in women with PCOS.
00:05:20
First thing, it's going to stimulate the ovaries to make more androgens like
00:05:23
testosterone.
00:05:25
It can disrupt ovulation which can lead to irregular or even absent periods.
00:05:30
It's going to promote fat storage, especially around the abdomen and the belly area.
00:05:35
And it can fuel the metabolic storm that makes PCOS so tough to manage.
00:05:40
And that a lot of conventional doctors are not going to know because they just focus on the
00:05:44
medication side of things and focusing on the symptoms, not on again, the entire person as a
00:05:50
whole.
00:05:51
They don't even think to look towards this insulin connection.
00:05:54
This is why so many women with PCOS are told to just lose weight, which is very frustrating and
00:06:00
very unhelpful actually, because we have to treat the insulin resistance first if we want
00:06:06
to reverse those symptoms.
00:06:07
So, the good news is that you can do this and through food, movement, supplements, sleep,
00:06:14
we've actually helped women to transform their PCOS outcomes naturally.
00:06:19
Now, the way we approach this, right?
00:06:21
We always want to talk about testing because if you're not testing, you're guessing.
00:06:26
And most doctors are not looking at the full picture, they're looking at a snapshot.
00:06:31
So, here's what I recommend you test if you suspect PCOS or if you want to track your
00:06:36
progress with this condition.
00:06:38
First things first, a full hormone panel.
00:06:41
We have to see total and free testosterone.
00:06:44
DHEA, which is an adrenal androgen.
00:06:47
We want to see LH and FSH, this is luteinizing hormone and follicle stimulating hormone.
00:06:54
If you have a high LH to FSH ratio, that's very common with PCOS.
00:06:59
We also want to check progesterone, and you want to check this mid luteal phase which is
00:07:03
about day twenty one of your cycle.
00:07:05
We also want to make sure we look at estradiol and sex hormone binding globulin, which is
00:07:10
often low in PCOS.
00:07:13
Now the next thing, right, we talked about how important it is the insulin component, we want
00:07:17
to check a metabolic panel, but not just glucose, right?
00:07:20
Most docs are just going look at your glucose and say okay, it's alright.
00:07:23
We want to check fasting insulin.
00:07:25
This one is huge, often skipped and probably the most important.
00:07:30
Then we want to check fasting glucose, hemoglobin A1C, which is that three month blood
00:07:35
sugar average, and a lipid panel.
00:07:37
We want to look at cholesterol and triglycerides as well.
00:07:40
And then, we want to look at a thyroid panel.
00:07:43
I want to see a full thyroid panel because these hormones play in tandem.
00:07:46
So, I want to see TSH, free T3 and free T4.
00:07:50
We want to see that reverse T3 and then we want to see antibodies, TPO and TG antibodies to
00:07:56
rule out if there is Hashimoto's, which is often found in connection with PCOS.
00:08:02
Some other helpful markers that we usually just have is ferritin, which is testing for the
00:08:07
body's storage of iron, vitamin D, magnesium, and HSCRP, which is high sensitivity C reactive
00:08:16
protein, which is a inflammatory marker that is often elevated in PCOS.
00:08:21
So, if you're working with a good functional medicine provider, like this person was that
00:08:26
sent me this message, these things should already be on their radar.
00:08:30
If not, advocate for yourself because you need the full picture.
00:08:34
And so, this is where you become your doctor and you can guide the process and ask for
00:08:38
things.
00:08:39
So, if you're listening to this, if you've been diagnosed with PCOS, if you suspect you may
00:08:45
have it, here are a few key strategies that you need to talk to your doctor about or start
00:08:51
implementing.
00:08:52
Number one, prioritizing blood sugar stability.
00:08:56
Protein with every meal, 20 to 30 grams minimum.
00:08:59
Cutting out entirely refined carbs and sugar, any processed food.
00:09:04
And then consider some time restricted eating or fasting if your body can tolerate it, if
00:09:08
you're not in a high stress environment.
00:09:11
Movement is key.
00:09:13
Walking daily, 10 steps a day.
00:09:16
And then adding in strength training, because building muscle will improve your sensitivity
00:09:22
to insulin.
00:09:23
Muscle is the most metabolic insulin sensitive organ that we have, and it can dramatically
00:09:29
lower insulin resistance.
00:09:31
The next thing is supporting your cycle.
00:09:33
There's some really great hormones that can help with this.
00:09:36
Magnesium is one.
00:09:38
Myo inositol, this is a fantastic one.
00:09:41
Also helps with balancing the thyroid with Hashimoto's.
00:09:45
N acetylcysteine and omega-3s, these can all help.
00:09:49
But if you don't know where to start or how much to take, be sure to work with a provider,
00:09:54
functional medicine doctor or someone who's qualified.
00:09:57
And then again, making sure you're running those right labs.
00:10:00
You want to check labs every three months when you're working with hormonal conditions because
00:10:03
so much can change in a short period of time.
00:10:06
That's the fasting insulin, the hormones, and then obviously the full metabolic picture.
00:10:12
And so, the takeaway here really is, you know, PCOS isn't just a fertility condition, it is
00:10:18
very much a metabolic and hormonal one.
00:10:21
And when we address it at the root, healing is absolutely possible.
00:10:26
So, I want to thank you for sending in this question.
00:10:29
Really glad I could go come on here and do this for you.
00:10:31
I hope this gave a little bit more clarity and encouragement to allow you to kind of take
00:10:35
action.
00:10:36
And you know, I want to let you know you're on the right path.
00:10:38
I'm cheering for you.
00:10:39
I think you're doing a lot of really good things.
00:10:41
And now how do we just take this a step further?
