Understanding your own body now pays dividends for the next thirty years.
00:00:05
50 is not the beginning of decline.
00:00:07
50 is the first time most women have enough data, enough life experience, enough self
00:00:12
awareness to actually do this right.
00:00:15
The women I see who optimize in their 50s are often performing at a higher level in their 60s
00:00:21
than they were in their 40s.
00:00:22
And that is not an accident.
00:00:24
Welcome to Health Decoded, your number one source for real health information with your
00:00:29
host, Doctor.
00:00:30
Alessi and Matt Tack.
00:00:36
Health Decoded listeners, if you're dealing with bloating that just won't go away no matter
00:00:41
what you eat, no matter what you try, I need you to hear this.
00:00:46
The bloating isn't the problem.
00:00:48
It's a signal.
00:00:50
Every external symptom you're experiencing, the discomfort, the fatigue, the frustration has a
00:00:56
root cause.
00:00:57
And in my years of clinical practice, it almost always leads back to the gut.
00:01:03
Nothing else in your body can fully heal until that foundation is restored.
00:01:08
I built this framework from my own health journey.
00:01:12
I was that patient, frustrated, dismissed, searching for answers.
00:01:17
I took everything I learned, refined it through years of clinical testing and have since helped
00:01:23
hundreds of patients achieve complete relief from their symptoms.
00:01:28
That became the Total Gut Reset, a step by step protocol with one goal: reset your gut in six
00:01:36
months or less.
00:01:37
If you're ready to learn exactly what's causing your symptoms and eliminate them for good, I'm
00:01:42
hosting a free webinar where I walk you through the entire framework.
00:01:47
The link is in the show notes.
00:01:49
Go ahead and register and I'll see you there.
00:01:53
I want to tell you about a patient that for sake of this episode, I'll call her Sarah.
00:02:00
Sarah was 52.
00:02:02
She ran a department of 40 people.
00:02:04
She was up at 05:30 every morning.
00:02:06
She was working out four days a week.
00:02:08
She was eating by most standards really well.
00:02:11
No junk food, no alcohol to speak of, lean protein, vegetables, the whole thing.
00:02:17
And she was exhausted.
00:02:19
She wasn't just tired.
00:02:20
She was like that dead exhausted type of feeling.
00:02:24
The kind of exhausted where you sit in the car after work for ten minutes before you can bring
00:02:28
yourself to go inside.
00:02:30
The kind where your brain stops cooperating around 2PM and you're running on caffeine and
00:02:35
willpower to get you through your day.
00:02:38
She had her blood work done twice.
00:02:40
Her doctor told her everything looked normal.
00:02:42
Her thyroid was fine.
00:02:44
Her CBC was fine.
00:02:45
Her metabolic panel was fine.
00:02:48
And then, because she's the kind of woman she is, she assumed the problem was just her.
00:02:53
Maybe she needed to sleep more.
00:02:55
Maybe she was just stressed.
00:02:56
Maybe this is just what 52 feels like.
00:02:59
When she came to see me, she was almost apologetic, like she was wasting my time.
00:03:05
She said, and I remember her saying this, I know I'm probably fine, I just don't feel like
00:03:11
myself anymore.
00:03:13
And I want to tell you what I told her.
00:03:15
You are not fine.
00:03:16
You are not sick either.
00:03:18
You are underperforming.
00:03:19
And those are completely different problems with completely different solutions.
00:03:24
By the time we finished her initial workup, we found a subclinical hypothyroidism that her
00:03:30
standard TSH had completely missed, a cortisol curve that had completely flattened, no morning
00:03:37
spike, no normal rhythm that we should see, and her gut was producing almost no short chain
00:03:43
fatty acids, which meant her mitochondria was basically running on fumes.
00:03:48
She wasn't broken, she was running this high performance engine on the wrong type of fuel
00:03:53
with a clogged filter and nobody had actually ever looked under the hood properly.
00:03:58
And that's what this episode is about.
00:04:00
If you've ever sat in your car after work and you've wondered when you stop feeling like
00:04:05
yourself, this is for you.
00:04:08
Welcome to Health Decoded.
00:04:09
I'm Doctor.
00:04:10
Alessi, founder and clinic director of Ascend Functional Health here in Tampa Bay.
00:04:16
This episode is gonna be part one of a four part mini series that I'm recording
00:04:21
specifically for the woman who is doing everything right by conventional standards and
00:04:26
still doesn't feel the way she knows that she should.
00:04:30
If that's you, stay with me for the next thirty minutes.
00:04:33
This changes how you think about your health permanently.
00:04:37
Let's get into it.
00:04:38
Let me start with a question.
00:04:40
What does healthy actually mean?
00:04:43
If you asked your conventional doctor, the answer is probably your labs are within normal
00:04:48
range, your blood pressure is acceptable, you're not presenting with any diagnosable
00:04:54
condition, congratulations, you're healthy, I'll see you next year at your checkup.
00:04:58
But that is the absolute floor.
00:05:01
That is the bare minimum definition of not sick.
00:05:04
And somewhere along the line, we've accepted that not sick and thriving are actually the
00:05:10
same thing, but they're not even close to the same thing.
00:05:13
Think about elite athletics for a second.
00:05:16
When a professional cyclist wants to optimize their performance, they don't go to their
00:05:20
primary care physician and ask if their CBC is normal.
00:05:24
They get a VO2 max test, they get lactate threshold testing, they get detailed hormone
00:05:30
panels timed to their training cycles, They look at mitochondrial function, they track
00:05:35
their sleep architecture, not just hours of sleep.
00:05:39
They optimize, they don't just survive.
00:05:41
Now why is that standard only for professional athletes?
00:05:45
Why is the woman who's running a business, raising a family, managing a household, still
00:05:51
trying to take care of herself, why is she expected to operate on a completely different
00:05:55
framework where the only bar is nothing's wrong, that's the gap.
00:06:00
And it's a very big gap.
00:06:02
The woman that I work with most, they're typically in their forties or fifties, high
00:06:07
functioning by external measure, and has a list of things that she's accepted as normal that
00:06:14
are absolutely not normal.
00:06:16
So let me read you some of them and this may be as a wake up call for a lot of people.
00:06:21
Waking up tired despite getting eight hours of sleep.
00:06:25
Carrying weight around the midsection that wasn't there ten years ago and it doesn't
00:06:29
respond to diet or exercise the way that it used to.
00:06:33
A brain that feels foggy or slow especially in the afternoon, a libido that has quietly
00:06:39
disappeared, joints that are achy, moods that feel flatter than it used to be, a sense that
00:06:46
your drive and motivation, which used to be your superpower, are running at about 60 now.
00:06:53
And she's been told these things are aging or stress or anxiety or you know, this is my
00:06:59
personal least favorite, that that's just what happens at this stage of life.
00:07:05
Now, if I had a dollar for every time somebody said that to me, it would be way too much.
00:07:11
I want to be very direct with you.
00:07:13
That is not aging.
00:07:15
That is underperformance.
00:07:16
And it has clinical causes that can actually be identified and addressed.
00:07:21
So let's get into some of that.
00:07:23
Let's get into what I call the five signals that you've learned to ignore.
00:07:27
I want to walk you through these performance signals and these are things your body is
00:07:32
telling you that you've probably been ignoring or worse just accepting at this point.
00:07:37
So signal number one, energy that doesn't recover.
00:07:41
Not tiredness after a hard day, I'm talking about a baseline fatigue that sleep doesn't
00:07:46
fix.
00:07:47
You got eight hours and you wake up unrefreshed.
00:07:50
You need caffeine just to get started and you hit a wall every afternoon.
00:07:55
That is not normal.
00:07:56
That is almost always driven by either HPA axis dysregulation, meaning your cortisol rhythm is
00:08:02
off, mitochondrial inefficiency, thyroid dysfunction or some combination of the three.
00:08:09
These things are measurable and yes, they are fixable.
00:08:13
Signal two: Body composition that resists effort You are doing the things, you're eating
00:08:19
well, you're exercising, but the scale won't move.
00:08:22
Or it moves and then it comes right back.
00:08:25
Or the body fat has redistributed to places that it never was before, particularly around
00:08:32
the belly.
00:08:33
That is a hormonal and metabolic signal.
00:08:36
Insulin resistance, cortisol elevation, estrogen dominance, thyroid insufficiency, any
00:08:42
and all of these changes where your body stores fat and whether it will release it are affected
00:08:48
by this.
00:08:48
And effort alone cannot override biochemistry.
00:08:53
Signal number three is that cognitive performance decline, brain fog, inability to
00:08:59
retrieve words, difficulty concentrating, feeling like you're operating through a layer
00:09:05
of glass.
00:09:06
Again, this is not aging.
00:09:08
This is almost always inflammation, gut brain axis disruption or hormone disruption.
00:09:14
Estrogen is neuroprotective and when it fluctuates or drops, cognitive clarity goes
00:09:20
with it.
00:09:21
Neuroinflammation, I just did an episode on this last week, is driven very often by
00:09:26
compromised gut barrier.
00:09:28
It impairs the very mitochondria that your neurons depend on.
00:09:33
Signal number four, sleep that doesn't restore.
00:09:37
So again, you might be getting hours of sleep, but not architecture.
00:09:41
Meaning you're not spending adequate time in deep sleep or REM.
00:09:45
This is where your brain consolidates memory and clears metabolic waste through the
00:09:49
glymphatic system.
00:09:51
If you're waking at two or 3AM and you can't get back to sleep, that's a cortisol spike.
00:09:56
And if you're sleeping through but you're waking exhausted, then it's often like a
00:10:01
progesterone deficiency or blood sugar dysregulation overnight.
00:10:04
And then the fifth signal that we see is a flat emotional baseline.
00:10:09
This is one of the hardest for high performing women to name because it's not depression, it's
00:10:14
not anxiety, it's more like a muting.
00:10:17
Things that used to excite you don't quite land the same way.
00:10:20
Your patience is shorter, your enthusiasm is lower, your drive, the thing that really makes
00:10:26
you who you are feels like it's at 60 or 70%.
00:10:30
That is often progesterone, testosterone and dopamine pathway dysfunction.
00:10:35
It's not a personality change, it's a biochemical one.
00:10:39
If you heard yourself in any one of those five, that's data.
00:10:43
That's your body communicating.
00:10:45
The question is whether you have someone in your corner who knows how to listen to it.
00:10:50
So let's get into what does it actually look like now to optimize versus just survive, just
00:10:57
manage a baseline of health.
00:10:59
And let me tell you what we're actually measuring when I sit down with a new patient
00:11:04
because it's very difficult from what you've probably ordered at your annual physical to
00:11:10
decipher these type of things.
00:11:11
A standard physical is gonna look at a CBC or a basic comprehensive metabolic panel.
00:11:18
Maybe they'll look at your TSH and maybe they'll look at a lipid panel.
00:11:22
That's usually it.
00:11:23
And that's what most physicians are using to determine whether you're healthy.
00:11:28
The problem is that none of those tests measure optimization.
00:11:32
They measure pathology and only gross pathology at that.
00:11:36
And so here's what a functional workup will actually look like.
00:11:40
On the hormonal side, I'm looking at a full thyroid panel, not just TSH.
00:11:46
TSH tells me your pituitary is sending a signal.
00:11:49
That's it.
00:11:50
It doesn't tell me that your cells are actually using thyroid hormone.
00:11:53
I want to see the free t three, free t four, reverse t three, and thyroid antibodies.
00:12:00
A woman can have normal TSH and be functionally hypothyroid.
00:12:04
And I see this all the time.
00:12:07
We also look at a full sex hormone panel, estradiol, progesterone, testosterone, DHEA,
00:12:13
sex hormone binding globulin.
00:12:15
Is your body creating and then actually utilizing the hormones Or is it creating them
00:12:20
and holding them because it's blocked?
00:12:23
And we look at cortisol and not just a single draw, but ideally a four point salivary
00:12:29
cortisol to see the actual diurnal rhythm.
00:12:32
Is it spiking in the morning?
00:12:34
Does it crash by noon?
00:12:36
Is it elevated at night when it should be low?
00:12:39
This rhythm matters enormously.
00:12:41
On the metabolic side, we'll see a fasting insulin, not just your glucose.
00:12:46
You can have perfectly normal blood glucose and be significantly insulin resistant.
00:12:51
I look at hemoglobin A1C.
00:12:53
I look at triglyceride to HDL ratio as a proxy for insulin sensitivity.
00:12:58
I look at homocysteine and high sensitivity CRP, and these are inflammatory markers.
00:13:04
We're going to assess gut function because the gut is the foundation of everything else.
00:13:10
Neurotransmitter production, immune regulation, hormone metabolism, nutrient absorption, all of
00:13:16
it runs through the gut.
00:13:17
We use a comprehensive stool analysis to look at the microbiome, at digestive enzyme
00:13:23
function, inflammatory markers in the gut itself.
00:13:27
Then, we look at micronutrient status because deficiencies in things like magnesium, zinc,
00:13:33
B12, folate, vitamin D, omega-three fatty acids, these all directly impair the systems
00:13:40
that I just described.
00:13:41
Most people are walking around deficient in multiple nutrients and have no idea because
00:13:46
they've never tested for them.
00:13:48
This is what a complete picture looks like.
00:13:50
When we have a complete picture, you can actually build a complete solution, not just a
00:13:54
guess.
00:13:55
Not a trial and error.
00:13:57
Not just here's some medication, let me know if it works.
00:14:00
This is a protocol built specifically for your biochemistry.
00:14:04
Sarah, the patient I told you about at the start, within ninety days of her personalized
00:14:08
protocol, she was waking up before her alarm feeling rested.
00:14:12
The afternoon crash was gone.
00:14:14
She lost somewhere around like eleven, twelve pounds without changing her exercise and with
00:14:19
only targeted dietary adjustments.
00:14:21
She told me at her follow-up that she felt like herself again for the first time in years.
00:14:26
And these are the stories that we're seeing all the time.
00:14:29
It's not a miracle.
00:14:30
This is what happens when you stop treating the floor and start building toward the ceiling.
00:14:36
And so there's an identity shift that needs to happen because we need to take the thought of
00:14:44
just surviving, just being a patient to optimizing your health, be able to carry your
00:14:50
body and carry you and your family and your loved ones for as long as possible.
00:14:55
Because isn't that the goal?
00:14:56
And so I really want to close with something that's more philosophical really than clinical
00:15:02
because I think it's actually the most important piece.
00:15:04
The conventional medical model is built around a reactive identity.
00:15:09
You're a patient.
00:15:10
You present with a problem, the system responds to the problem, you go home, and you wait for
00:15:16
the next problem.
00:15:17
That model is not built for you.
00:15:20
It is not built for the woman who wants to be exceptional at 55 the way she was at 35.
00:15:25
It is not built for longevity.
00:15:27
It is not built for performance.
00:15:29
It's built for crisis management.
00:15:31
The identity I work with the one I want every patient at Ascend Functional Health to carry,
00:15:37
is that of an optimizer someone who understands that their biology is a system and that systems
00:15:44
can be measured, supported and improved.
00:15:47
That the investment you make is understanding your own body now pays dividends for the next
00:15:52
thirty years.
00:15:54
50 is not the beginning of decline.
00:15:56
50 is the first time most women have enough data, enough life experience, enough self
00:16:01
awareness to actually do this right.
00:16:04
The women I see who optimize in their 50s are often performing at a higher level in their 60s
00:16:10
than they were in their 40s.
00:16:11
And that is not an accident.
00:16:13
That's an intentional decision.
00:16:15
The question is not whether you can feel better than you do right now, you absolutely can.
00:16:21
The question is whether you have the right person, the right people, and the right
00:16:25
accountability to help you get there.
00:16:28
And so, over the next three weeks, stay tuned.
00:16:31
I'm going be releasing three more episodes in this series.
00:16:34
We're going to talk about the real conversations of taking your health seriously
00:16:39
and what inaction is actually costing you.
00:16:42
We're going to break down clinically why the things you've already tried haven't produced
00:16:46
lasting results.
00:16:47
And we're going to talk about what thriving in your fifties, sixties and beyond actually looks
00:16:52
like and how you get there.
00:16:54
So if you're listening to this and something landed with you, if you heard yourself in the
00:16:58
five signals that I mentioned, if Sarah's story sounded familiar to you, I want to hear from
00:17:03
you.
00:17:04
You don't have to be sick to come to Ascend Functional Health.
00:17:06
You don't have to have a diagnosis.
00:17:08
You just have to be done accepting less than what your body is capable of.
00:17:12
So you can always book a discovery call at the link in the show notes.
00:17:17
If you have lab tests already done, bring them with you.
00:17:20
If you don't, we can start fresh.
00:17:22
Either way, we're gonna get you a complete picture and a real plan.
00:17:26
Again, I'm Doctor.
00:17:27
Alessi.
00:17:28
Thank you for joining me on Health Decoded, and I'll see you next week.
