Primary drive is not thinness, it's safety and survival.
00:00:05
When the body is inflamed, when the gut barrier is compromised, when the liver is overburdened,
00:00:10
when hormones are dysregulated because the systems that process and clear them are backed
00:00:15
up, the body will be in a defense state.
00:00:18
It's allocating resources towards management of perceived internal crisis.
00:00:24
Fat storage in that context is not a character flaw.
00:00:27
It's often a physiological strategy.
00:00:30
Welcome to Health Decoded, your number one source for real health information, with your
00:00:35
hosts, Doctor.
00:00:36
Alessi and Matt Tack.
00:00:41
Health Decoded listeners, if you're dealing with bloating that just won't go away no matter
00:00:47
what you eat, no matter what you try, I need you to hear this.
00:00:52
The bloating isn't the problem.
00:00:54
It's a signal.
00:00:55
Every external symptom you're experiencing, the discomfort, the fatigue, the frustration has a
00:01:02
root cause.
00:01:03
And in my years of clinical practice, it almost always leads back to the gut.
00:01:08
Nothing else in your body can fully heal until that foundation is restored.
00:01:14
I built this framework from my own health journey.
00:01:17
I was that patient, frustrated, dismissed, searching for answers.
00:01:23
I took everything I learned, refined it through years of clinical testing, and have since
00:01:28
helped hundreds of patients achieve complete relief from their symptoms.
00:01:33
That became the Total Gut Reset, a step by step protocol with one goal, reset your gut in six
00:01:41
months or less.
00:01:42
If you're ready to learn exactly what's causing your symptoms and eliminate them for good, I'm
00:01:48
hosting a free webinar where I walk you through the entire framework.
00:01:52
The link is in the show notes.
00:01:54
Go ahead and register, and I'll see you there.
00:01:59
Welcome back to the Health Decoded podcast.
00:02:02
I'm glad you're here today.
00:02:03
My name's Doctor.
00:02:04
Alessi, and today we're going to talk about something that sits at the very core of what we
00:02:10
do.
00:02:11
This is a program that a majority of our patients go through often as the very first
00:02:16
thing.
00:02:18
It's called our repair and clear program.
00:02:21
And I want to be very upfront with you.
00:02:23
This episode is not a sales pitch.
00:02:25
This is me trying to explain because too many people come to us having tried a million things
00:02:31
that didn't work.
00:02:33
And the reason they didn't work is that no one ever explained why we do things in a specific
00:02:38
order or what's actually happening in the body when we do them.
00:02:43
So, today, I'm going to walk through the science, three distinct phases, six weeks, and
00:02:50
a framework for understanding why when you stop fighting your body and start supporting it,
00:02:56
everything starts to work better.
00:02:58
Including, yes, your weight and weight loss.
00:03:01
So, let's get right into it.
00:03:03
The first piece is, I want to talk about why detox, and I'll air quote that, is a real thing
00:03:11
and really why the word got ruined and watered down.
00:03:15
So, let's start by really getting into, you know, the second you hear the word detox, most
00:03:21
people are gonna picture a juice cleanse that someone's selling on Instagram or some three
00:03:26
day fast with cayenne pepper water.
00:03:29
That's not what I'm talking about.
00:03:30
That version of detox is largely theater.
00:03:34
What we're talking about is actual physiologic physiological change.
00:03:38
And here's the real concept.
00:03:40
Every single day, your body is exposed to compounds that it has to process and eliminate.
00:03:46
Some of these come from the outside.
00:03:49
Pesticide residue on food, chemicals in your personal care products, pollutants in the air,
00:03:55
medications you may be on, food additives.
00:03:59
Some are generated internally such as byproducts of normal cellular metabolism,
00:04:05
hormones that need to be broken down and cleared, and inflammatory compounds.
00:04:10
Your body has an extraordinary built in system for handling this.
00:04:15
The liver is the primary processing plant.
00:04:18
The kidneys are the filters.
00:04:21
And then the gut is the elimination piece.
00:04:23
Your skin is also involved through sweating.
00:04:26
And this isn't fringe medicine, this is actually just standard human physiology.
00:04:32
The problem isn't that the system doesn't exist, the problem is that the system can get
00:04:37
overwhelmed.
00:04:38
And when it does, it doesn't just stop working, it slows down.
00:04:43
It backs up.
00:04:44
Compounds that were supposed to be processed and expelled stay in circulation longer than
00:04:49
they should.
00:04:50
Some get stored in fat tissue and connective tissue because the body is essentially saying,
00:04:56
I'll deal with this later.
00:04:58
That later does have a cost.
00:05:01
When the liver's detox pathways are sluggish, you see it in the energy levels, hormonal
00:05:06
imbalance, skin issues, brain fog, and stubborn weight that doesn't move despite reasonable
00:05:13
effort.
00:05:14
These are not separate problems.
00:05:16
They're often symptoms of the same underlying bottleneck.
00:05:20
Now, here's where it gets interesting.
00:05:22
The liver processes toxins in two distinct chemical stages called phase one and phase two
00:05:29
detoxification.
00:05:31
Phase one converts fat soluble toxins into intermediate compounds using enzymes.
00:05:38
Phase two then takes those intermediates and binds them to other molecules such as amino
00:05:45
acids, sulfur compounds, glutathione to make them water soluble so that your body can
00:05:52
excrete them.
00:05:53
Here's the catch.
00:05:54
If phase one is running but phase two is sluggish, then the phase one intermediates,
00:06:00
which are often more reactive than the original toxins themselves, will begin to accumulate.
00:06:05
This is actually worse than if you hadn't started the process at all.
00:06:09
This is why you can't just throw supplements at somebody and call it a detox because sequences
00:06:14
and support matter.
00:06:16
Our Repair and Clear program is built around that reality.
00:06:20
It's designed to support both phases in sequence in a way that the body can actually
00:06:25
use.
00:06:27
But before we even get to the liver, you have to talk about the gut.
00:06:30
Because the gut is not just the front door, it's the security system.
00:06:35
The gut is your body's first line of defense and where most programs start too late.
00:06:41
So, let me describe what your gut is actually doing.
00:06:45
Because most people have simplified the picture of it.
00:06:48
Food goes in, nutrients come out, waste gets excreted.
00:06:53
While that's true, it's a massive understatement.
00:06:56
The gastrointestinal tract is a 25 foot long tube lined with a single layer of epithelial
00:07:02
cells.
00:07:04
These are called mucosal cells.
00:07:06
These cells are basically your border control.
00:07:09
They decide what gets absorbed into the body and what doesn't.
00:07:13
The connections between these cells are called tight junctions and they're supposed to be, as
00:07:18
the name implies, tight.
00:07:21
Only small, properly digested nutrient particles pass through under normal
00:07:25
circumstances.
00:07:27
But here's what happens when that barrier is compromised.
00:07:31
Those tight junctions begin to loosen and when they loosen, things that have no business
00:07:37
getting into your bloodstream, such as partially digested food, bacterial fragments,
00:07:43
inflammatory compounds, they start getting through.
00:07:46
Your immune system, which is heavily concentrated in the gut lining, encounters
00:07:51
these foreign particles and begins to mount a response.
00:07:55
This isn't just a minor inconvenience.
00:07:57
That response, that chronic low grade activation is now understood to be a major
00:08:03
driver of systemic inflammation.
00:08:06
And systemic inflammation is implicated in everything from autoimmune conditions to
00:08:11
metabolic dysfunction to mood disorders and anxiety.
00:08:15
The gut lining also plays a direct role in liver burden.
00:08:20
Every substance absorbed from the gut travels first to the liver through what's called the
00:08:25
portal circulation.
00:08:27
So if the gut barrier is compromised and letting through compounds it shouldn't, the
00:08:32
liver is the first organ to take the hit.
00:08:34
This is why we always address the gut before we address the liver.
00:08:39
You cannot effectively support liver detox if the gut is still dumping inflammatory compounds
00:08:44
into the portal circulation.
00:08:46
You're basically filling the bucket faster than you can empty it.
00:08:50
So weeks two and three of our Repair and Clear program are dedicated entirely to this.
00:08:55
The intestinal support phase we call it.
00:08:58
The dietary protocol during this phase is strict and specific.
00:09:02
We eliminate the primary known dietary triggers: gluten, dairy, soy, refined sugars,
00:09:09
alcohol, nightshade vegetables, legumes, and most grains.
00:09:15
Not because we believe everyone is permanently intolerant to all of these, but because these
00:09:19
are the most common contributors to intestinal irritation and immune activation.
00:09:25
And when someone's gut is already compromised, we need to remove every variable we can while
00:09:30
we're actively supporting the lining's recovery.
00:09:32
What you eat during this phase is equally important.
00:09:35
Organic vegetables, quality proteins, fermented foods to support your microbiome, healthy fats,
00:09:43
and you eat frequently.
00:09:44
This isn't a fasting program because fasting actually elevates cortisol and cortisol is
00:09:50
catabolic to the gut lining.
00:09:52
We're trying to repair, not stress.
00:09:55
Blood sugar stability during this phase is not minor detail either.
00:09:59
The microbiome, is the community of bacteria, fungi and other organisms living within your
00:10:06
gut, this also factors heavily here.
00:10:09
A healthy gut microbiome does things that your own cells cannot.
00:10:13
It synthesizes vitamins, it produces short chain fatty acids that fuel the gut lining
00:10:19
itself and it competes against pathogenic organisms for real estate.
00:10:24
When the microbiome is disrupted by poor diet, antibiotic use, chronic stress or infections,
00:10:31
that competition gets lost.
00:10:34
So, supporting microbial diversity during this phase is a non negotiable part of the approach.
00:10:40
By the time someone finishes weeks two and three, the goal is a gut lining that is better
00:10:46
sealed, less inflamed, and not actively dumping fuel onto the liver's fire.
00:10:52
And so now, we're ready to talk about the next phase which is the liver support phase.
00:10:57
This is where detoxification gets done right.
00:11:00
And so that's when you proceed into weeks four, five and six where we're supporting the liver.
00:11:05
And the diet shifts here in a very deliberate way.
00:11:10
Again, this program is a combination of things strung together in a sequence.
00:11:15
And so the five stage dietary progression of this phase is designed to do something very
00:11:20
specific.
00:11:21
It gradually modulates the load on the detox pathways while systemically supporting the
00:11:28
nutrients and co factors those pathways require to function.
00:11:32
Remember when I said earlier about phase one and phase two?
00:11:36
So phase one is enzyme driven.
00:11:38
It uses a family of enzymes called cytochrome P450 enzymes and phase two requires specific
00:11:46
amino acids and micronutrients to run these conjugation reactions.
00:11:51
Things like glutathione, taurine, glycine, sulfur compounds, these are the raw materials.
00:11:58
If you run phase one without adequate phase two support, you create more reactive intermediates
00:12:04
and more oxidative stress.
00:12:07
The dietary protocol during this phase is designed to ensure those raw materials are
00:12:12
available.
00:12:14
The phase structure moving from a broader allowed food list through a more restricted
00:12:20
window and then back out is intentional.
00:12:23
It allows the body to mobilize stored toxins, process them through pathways that are now
00:12:29
being actively supported, and then excrete them.
00:12:32
It's not arbitrary.
00:12:33
It actually mirrors how the biology is supposed to work.
00:12:37
There's also an important point about fat tissue here.
00:12:41
Many persistent toxins, environmental chemicals, hormonal metabolites, certain
00:12:46
pharmaceutical byproducts are lipophilic, meaning they're fat soluble.
00:12:51
They accumulate in adipose tissue specifically because the body uses fat as a long term
00:12:57
storage depot for compounds that it can't immediately process.
00:13:02
When someone loses weight, they release these stored compounds back into circulation.
00:13:07
This is well documented in literature and it's one reason why rapid aggressive weight loss can
00:13:13
sometimes make people feel worse, not better.
00:13:16
If the detox pathways aren't supported during that process, you're liberating stored toxins
00:13:22
faster than the liver can handle them.
00:13:24
This is a key piece of our philosophy around weight loss, which I want to actually address
00:13:29
real quick.
00:13:31
So, the weight loss conversation that we actually need to talk about is people come to
00:13:36
us wanting to lose weight.
00:13:37
That's true.
00:13:38
And it's a legitimate goal, but I want to challenge you on the framing most people bring
00:13:44
with them.
00:13:45
The conventional model of weight loss treats the body like a simple accounting equation.
00:13:51
Calories in, calories out, adjust the deficit.
00:13:54
And while energy balance is real and it does matter and it is something we address, that
00:14:00
model alone fails to explain why two people can eat the same diet and get dramatically
00:14:05
different results.
00:14:06
Because it fails to explain why someone can be doing everything right and still not lose
00:14:12
weight.
00:14:13
And it definitely fails to explain why some people have metabolic resilience that seems to
00:14:18
evaporate once their health deteriorates.
00:14:21
Here's how we think about it.
00:14:22
If the body's primary drive is not thinness, it's safety and survival.
00:14:28
When the body is inflamed, when the gut barrier is compromised, when the liver is overburdened,
00:14:33
when hormones are dysregulated because the systems that process and clear them are backed
00:14:38
up, the body will be in a defense state.
00:14:42
It's allocating resources towards management of perceived internal crisis.
00:14:47
Fat storage in that context is not a character flaw.
00:14:51
It's often a physiological strategy.
00:14:53
The body is storing excess lipophilic toxins in the fat.
00:14:57
It holds on to weight as a buffer when cortisol is chronically elevated.
00:15:02
And thus, insulin resistance, is the impaired ability to get glucose into the cells, is
00:15:08
deeply intertwined with inflammatory signaling.
00:15:12
When inflammatory cytokines block insulin receptor function, you get elevated blood
00:15:18
glucose and a body that is increasingly efficient at storing fat and inefficient at
00:15:23
burning it.
00:15:25
So, the question we ask is not how do we force the body to lose weight?
00:15:29
The question is what is the body defending against and how do we remove that burden so it
00:15:34
no longer needs to defend?
00:15:36
When you seal the gut, when you reduce dietary inflammatory triggers, when you support the
00:15:42
liver's ability to clear accumulated compounds and process hormones efficiently, when you
00:15:48
bring down systemic inflammation, the body's threat perception drops.
00:15:53
Cortisol regulation improves, insulin sensitivity improves, the hormonal signals that
00:15:59
govern appetite and fat metabolism, such as leptin, ghrelin, and adiponectin, they start
00:16:04
working the way they're supposed to.
00:16:07
Weight loss becomes a downstream effect of a body that is functioning well, rather than a
00:16:12
battle fought against a body that has poor function.
00:16:17
And so, I'm not promising that this program is a weight loss program.
00:16:22
What I'm saying is that for patients who struggled with weight despite reasonable
00:16:26
effort, removing the underlying physiological burden is often what finally allows for
00:16:32
meaningful change to happen.
00:16:34
And that's a fundamentally different conversation than just counting calories.
00:16:39
And you know, the main piece that most programs are going to skip entirely is what I wanna talk
00:16:44
about next, is the the the intro week or what we call like the priming week.
00:16:50
Because I've mostly been talking about weeks two through six, which is the gut and the
00:16:54
liver.
00:16:54
But I wanna go back to the first week because it's actually my favorite part of this program
00:16:59
and it's where most people will overlook.
00:17:03
Week one is what we call the baseline immune support phase.
00:17:07
We don't implement any dietary restrictions in this phase.
00:17:10
The focus is on reducing systemic inflammation and priming the immune system before we ask the
00:17:16
gut and the liver to do a bunch of hard work.
00:17:19
Why does this matter?
00:17:20
Well, think about what we're asking the body to do.
00:17:23
We're about to impose significant dietary restrictions, mobilize stored toxins, and put
00:17:29
the gut and liver through a supported but demanding process.
00:17:33
If you walk right into that while the immune system is dysregulated and inflammation is
00:17:38
already high, you're starting in a deficit.
00:17:41
There's a concept in functional medicine and exercise physiology called readiness.
00:17:46
Before you train, you warm up.
00:17:49
Before you impose a physiological demand, you need to prepare the system.
00:17:53
And week one is that preparation.
00:17:56
We use specific things turmeric, resveratrol, vitamin D.
00:18:00
These are not random choices.
00:18:02
Curcumin, the active compound in turmeric, is one of the most extensively studied natural
00:18:07
compounds for its role in modulating NFKB, which is a master regulator of inflammatory
00:18:14
signaling.
00:18:16
Resveratrol supports mitochondrial function and has demonstrated activity in supporting
00:18:21
cellular resilience.
00:18:23
Vitamin D is not just a vitamin, it's actually more like a hormone with receptors throughout
00:18:28
the immune system, the gut epithelium and the liver.
00:18:31
Chronic vitamin D insufficiency is endemic in our population and actually has documented
00:18:37
effects on both gut barrier integrity and immune regulation and that's why it's a key
00:18:43
piece of everything we do.
00:18:45
By the time someone finishes their first week and moves into the intestinal support phase,
00:18:50
the inflammatory environment has been modulated, the immune system has more support
00:18:54
behind it, and the program's downstream phases are more likely to work.
00:19:00
For patients on our comprehensive plan, there are additional supports for antioxidant defense
00:19:05
during this week.
00:19:06
Glutathione pathway is specifically enhanced.
00:19:09
Glutathione is your body's master antioxidant and it's central to that phase two liver detox.
00:19:16
Proactively supporting its availability before the detox phase begins is just based on good
00:19:21
science.
00:19:23
Now, you're probably wondering, is this for me?
00:19:26
Who should do this?
00:19:28
And who should actually not do this?
00:19:31
Maybe have a conversation first because again, it's not a one size fits all.
00:19:36
But before I close, let me say something practical.
00:19:39
This program is foundational to our practice because the underlying physiology it addresses
00:19:45
things like gut barrier function, liver detox capacity, systemic inflammation, microbiome
00:19:51
health.
00:19:52
These are issues that are relevant to almost every chronic health concern that we see:
00:19:57
autoimmunity, hormone imbalance, fatigue, cognitive impairment, metabolic dysfunction and
00:20:03
weight resistance.
00:20:04
But, relevant to almost everyone is not the same as right for everyone right now.
00:20:10
If you're on medications, this program does not replace them and it's not designed to.
00:20:15
We run this alongside your existing care.
00:20:18
The dietary changes are significant and should be disclosed to any provider that you're
00:20:23
obviously working with because dietary changes can affect medication absorption and
00:20:27
efficiency.
00:20:28
So, if you have a history of significant digestive conditions, Crohn's, ulcerative
00:20:33
colitis or severe gastroparesis, the specifics of how we run this program may need to be
00:20:39
modified.
00:20:40
And so that's a conversation, not a barrier, it's just we need more information.
00:20:44
If you're pregnant or breastfeeding, not the time for this type of program because mobilized
00:20:49
toxins can cross the placental barrier, enter breast milk, and that's a risk we just don't
00:20:54
take.
00:20:55
If you've tried restrictive programs before and found that they were psychologically
00:21:00
destabilizing, That matters to us because it's worth preparing and getting ready for something
00:21:05
like this.
00:21:07
The dietary phase of this program is strict intentionally, but it's not about restriction
00:21:12
just for that piece.
00:21:14
Having clarity on why is going to make a significant difference in completing something
00:21:19
like this.
00:21:20
What I will say is this, in over a decade of clinical practice and functional medicine, this
00:21:25
six week program is the one that we return to consistently because the results are
00:21:30
consistent.
00:21:31
Patients come out on the other end with better energy, clear thinking, improved digestion,
